ARTICLE 19: SEC: 903
A health certificate shall be furnished by each pupil in the public school within 30 days of his / her entrance in such schools, and by each child entering Kindergarten, second, fourth, seventh, and tenth grade thereof. An examination and health history of any child may be required by the local school authorities at any time in their discretion to promote the educational interests of such child. Each certificate shall be signed by a duly licensed physician, physician assistant, or nurse practitioner, and shall describe the condition of the pupil when the examination was made, and state whether such pupil is in a fit condition of bodily health to permit his or her attendance at the public schools. If such pupil does not present a health certificate as herein required, unless he or she has been accommodated on religious grounds, the principal or teacher in charge of the school, shall cause a notice to be sent to the parents of such pupil that if the required health certificate is not furnished within 30 days from the date of such notice (at registration), an examination will be made of such pupil by the school physician.

Pre-K, Nursery, Daycare

3 Diphtheria, 3 (IPV, OPV or any combination of IPV or OPV), 1 Measles, 1 Mumps, 1 Rubella, 3 Hib
(or 1 Hib if administered
> 15 months of age), DSS licensed daycare centers also require Tetanus
and Pertussis vaccinations, 3 Hepatitis B, 1 Varicella for children born on or after 1/1/2000.

K-12 (born before 1985)

3 Diphtheria, 3 OPV 1 Measles, 1 Mumps, 1 Rubella

K-12 (born on or before 1/1/1985)

3 Diphtheria, 3 (IPV, OPV or any combination of IPV and OPV) 2 Measles, 1 Mumps, 1 Rubella,
3 Hepatitis B for children born on or after 1/1/94, 1 Varicella for children born on or after 1/1/98

and students born on or after 1.1.94 entering (repeating or transferring into 6th grade and students
in gradeless classes) born on or after 1/1/94 and age equivalent to the 6th grade.

College (born on or after 1/1/57)

2 Measles, 1 Mumps, 1 Rubella
