St. Helen's School 2008-2009 Medical Letter

Dear Parents or Guardians:

Welcome to St. Helen's School. I would like to share some information with you regarding New York State Education laws and regulations regarding the health requirements for new students entering school.

1. All new students must have a physical examination done by their own physician or the school physician. The physical exam shall not be more than twelve months prior to the commencement of the school year.

2. A record of immunizations is required for admittance to school. These should include:
- 3 doses of diptheria toxoid, 4 recommended (DPT, DT, Td)
- 3 doses of oral poliovirus vaccine, 4 recommended
- 1 dose of mumps vaccine
- 1 dose of rubella (German Measles) vaccine
- 2 doses of the measles vaccine
- 3 doses of the hepatitis B vaccine
- 1 dose of the varicella vaccine (chicken pox) or documentation of having had varicella from a physician
