FOR CHILD #1             Plan 1   I commit to paying $3,200 for my child and I agree to participate in fundraising. I understand that the per pupil cost of educating a child at Saint Helen's School is $5,100 and I know that I cannot pay that amount. I accept an automatic tuition assistance package of $1,900.             Plan 2   I commit to paying a reduced tuition and agree to participate in fundraising. I understand that the per pupil cost of educating a child at Saint Helen's School is $5,100 and I know that I cannot pay that amount. I accept an automatic tuition assistance package of $1,900 AND I need to apply for additional tuition assistance. Please send me an application for additional tuition assistance. I understand that this application has to be mailed by 4/15/07 to FACTS Tuition Management Services in order to be considered for the Lally and Beacon of Hope Scholarship Funds or for in-school tuition assistance.             Plan 3   I commit to paying $5,100 for my child. I understand that the per pupil cost of educating a child at Saint Helen's School is $5,000 and I know that I can pay that amount.     I would still like to be involved in fundraising and school sponsored events.     I do not wish to be involved in fundraising or school sponsored events.             If you will have more than one child at Saint Helen's in K-5: Choose Plan 1, or 3 above for the first child and then complete the following for Child 2 and Child 3.             CHILD #2       Plan 1   I will accept tuition assitance in the amount of $2,750 and pay $2,350 ($5,550 for two students). Plan 3   I choose not to accept tuition assistance, and pay $5,100 per pupil.             CHILD #3 Plan 1   I will accept tuition assitance in the amount of $3,300 and pay $1,800 ($7,350 for three students).
